How It Works
It is actually very simple. In the past, the collection process was driven by multi-part, hand-written forms that were susceptible to missing or incomplete information. They also left the employer in the dark about much of the collection process.
Our new system simplifies the collection process. It incorporates a revolutionary (patent applied for) new form with an online system. The new universal form combines donor information, a donor receipt and the bar coded specimen labels into a single page.*
- The form comes as blank stock with only a forensic safeguard pre-printed on it. The collection process is handled primarily online, until the collection or test has been ordered and performed.
- The appropriate information is printed onto the form, labels are put in place and initialed, donor receipts are separated and distributed and the form packaged with the sample and sent to the lab.
- Employers can now monitor the status of their order through our "report status" feature. The system will report the status at certain check points, such as "donor checked-in", "in-process", "refused" or "completed" and "test results" (positive, negative, pending) etc.
DOT collections are supported using a dot-matrix printer, which will allow you to print the Federal forms using FormFox.
